Contributions through per service fees, which are paid before each appointment, can be done in-person at the front desk or online by clicking the "choose your own amount" button. Annual fees can be paid either online, through this page, or in-person.
Annual fees are based on patient income for the year. Fees are determined for each patient based on what percent of the Federal Poverty Limit (FPL) their income is.
For individual patients*:
0-100% FPL = income of $0-$12,760
101-150% = income of $12,887.60-$19,140
151-200% = income of $19,267.60-$25,520
201-250% = income of $25,647.60-$31,900
251-300% = income of $32,027.60-$38,280
*FPL values are different for multiple people in a household. Check the bottom of the patient eligibility tab on the Community Care Clinic Website to see FPL values for patients with multiple members in the household.
If you are unsure what your FPL value is or have questions, please call the clinic at 252-261-3041. For the staff at the clinic to determine your FPL, they will need your yearly income and the number of individuals in your household.
